Solid mass arising from the body of the testes
-> Urgent USS. If malignancy confirmed test serum α-FP, βhCG & LDH and refer 2ww

Scrotal masses not arising from the body of the testes
Where diagnosis is uncertain -> routine USS

Varicocoele
Refer for renal USS to exclude renal mass

Paediatric undescended testes
Do not require ultrasound confirmation and can be referred to the surgeons as per the guidelines here

Haematospermia
Does not require investigation with USS unless associated scrotal mass

Pain
Acute onset requires urgent referral to urology to exclude torsion
USS is rarely helpful in cases of chronic pain in the absence of a palpable scrotal mass
