Mental Health Referral Pathway
Email referrals are the preferred method. All emails will be monitored Monday – Friday between the hours 9.00am – 17.00pm, by a referral administrator. All referrals are managed within 24hours of receipt. If the referral is urgent please ensure that this is clearly stated in the email subject title so that it can be prioritised.
All referrals are triaged by a qualified mental health professional. The patient will be contacted by telephone and a triage assessment will be completed prior to an appointment.
(Please note this triage assessment may result in a change in the referral priority.)
Please complete the attached referral form; we require all sections to be completed in order to complete an effective triage assessment. Please provide us with your contact details so that we can provide feedback on the assessment outcome. All assessment feedback will be sent to the referrer, GP & Patient.
When considering a referral to the ICMHT, please refer to the assessment timescales:-
Same day assessment- please contact the assessment lead to discuss level of urgency and if same day assessment is required by ICMHT or HTT.
Urgent– patients will be seen within 5 working days.
Routine– patients will be seen within 28 working days.
If you require an assessment outside of the ICMHT working hours a weekends please contact the Out of Hours Team for advice and consultation.
The locality model for the ICMHT’s is as follows:
North & East– Caradon & North Cornwall area.
Mid– Restormel & Carrick area.
West & Isles of Scilly– Kerrier, Penwith & Isles of Scilly.

Please ensure that the patient has agreed to the referral to the ICMHT as we will be unable to proceed with triaging the referral if the patient is unware. If the patient refuses a referral to the ICMHT please consider the patients capacity to make an informed decision to consent to the referral. If a Mental Health Act is required please contact the Out of Hours Team.

Out of Hours - Home Treatment Team
The Home Treatment Team have extended their working hours until 10.30pm every day instead of 8.30pm
This will enable the team to offer increased evening visits, provide medication drops and respond more quickly to requests for urgent Mental Health assessments.
All GP surgeries and out of hours GPs including 111 will have access to a direct professional line into HTT for urgent referrals during the out of hours period.
