Work Well
This guideline applies to adults aged over 16 years.
Introduction
Work Well is a service that helps people with a health condition or disability to start, stay and succeed in work.
Referral
Work Well provides an early intervention work and health assessment and provision of low intensity support, tailored to the individual to
- address their work and health needs
- overcome health barriers at work
- access help, support, information, guidance and connections to services
- provide phone or face to face work and health coaching if required
It is a prevention service aimed to support those most recently out of work due to sickness, those returning to work and those at work who are at risk of being signed off sick.
Work Well is accessed via the Kernow Community Gateway
Kernow Community Gateway is open 08:00 – 20:00, 7 days a week, 365 days a year.

Patients can self refer on 01872 266383 (open 08:00 – 20:00 365 days a year)
If lines are busy, please leave a message and the community gateway will call back
Health care professionals and their wider social prescribing team can also make a referral with the patient’s consent by any of these options:
