Kernow Community Gateway
This guideline applies to adults aged 18 and over, with the exception of Work Well which is for adults over 16 years.
Introduction
Kernow Gateway is a simple referral route to access a range of voluntary, community and independent sector support services.
It is a dedicated telephone line available for everyone and provides access to a wide range of professional voluntary sector support.
Open 08:00 – 20:00, 7 days a week, 365 days a year.
It also has access to multi agency support workers in the community to provide one to one and group support at place.
Referral
The Kernow Community Gateway team provide:
- Guided what matters to you conversations
- Helps people with a wide range of needs, such as social, emotional, financial, employment, physical and mental health and social isolation
- Personalised support based on what matters to the person
- Develops personalised support and action plans to prevent hospital admissions, reduce social isolation and improve wellbeing
- Connects people to community hubs, peer support, mental health support, pain cafes, step into wellness programmes, falls prevention, carer support and more
- Helps people make sense of support options and pathways
- Supports people who are self funding to source personal care
- Provides WorkWell work and health coaches that:
- assess and provide low intensity support tailored to people’s health needs affecting work
- help overcome work barriers
- help access help, support, information, guidance and connections to services
- provide phone or face to face work and health coaching if required

Kernow Community Gateway do not provide an urgent or emergency response
They do not personally deliver or source personal care
They do not provide case management

If lines are busy, please leave a message and the community gateway will call back
Health care professionals and their wider social prescribing team can also make a referral with the patient’s consent by any of these options:
