Weight Management in the Community

 

Introduction

Several public health initiatives provide long-term support and guidance for weight management patients.

 

Available Services:

3. Healthier You for Pre-Diabetes

  • Referral Criteria: Fasting blood glucose 5.6-6.9 mmol/l or HbA1c : 42-47.9 mmol/mol
  • Referral Process: Self-referral via above link
  • Further Details: Information and support to prevent developing type 2 diabetes, 43-week programme (9 months)
  • Initial discovery call (30 mins), then 13 group sessions (2 hours), Additional support: Private Facebook group peer-support, option of progress calls, access to online gym/exercise sessions.

5. Oviva

  • Medical weight loss - covered by the NHS | Sign up with Oviva
  • Referral Criteria: Over 18 years and meet one of the below criteria:
    • BMI>50
    • BMI at least 40 with failed weight loss over last two years having engaged with other community weight loss services
    • BMI at least 35, with one weight related co-morbidity and failed weight loss over last two years having engaged with other community wight loss services
  • Some exclusion criteria apply – please see referral form on website for details.
  • Referral Process:
    • GP surgery staff: refer via e-RS, or email referral to ovivauk.t3wm@nhs.net
    • Referral template available to download from Oviva website above, or alternatively via Ardens local document library (where commissioned).
  • Further Details:
    • Fully remote, NHS Tier 2 weight management service.
    • Oviva offers a weight loss programme available on the NHS led by experienced doctors. It lasts 6-24 months, depending on preferences and eligibility for medications. The programme covers everything from eating habits, mental health, to exercise routines and nutritional needs.

7. Community dietetics

  • The community dietetic team offer a one-off advice and guidance appointment if required.
  • Community dietetic weight management referrals will not be accepted unless there is a specific reason the other options cannot be accessed.
