This guideline applies to adults aged 18 years and over.

Telephone advice:
Professionals Palliative and End of Life care advice and admissions line: 01736 757 707 (24hrs, 7 days a week). Note Specialist palliative consultant advice remains available 24 hours with cover provided by Supportive Care UK between 5pm-9am.
This is for:
- Requesting in-patient admission to Cornwall Hospice Care
- Healthcare professional advice in any setting.
Note:
- This is not an emergency response service and a proactive approach to end-of-life care is best.
- Patients should have a timely review before seeking specialist consultant advice.

- Email:cpn-tr.spcreferral@nhs.net (weekdays 9am-4.30pm only) with completed referral form

- Management and monitoring of persistent and/or transient symptoms
- Management of complex emotional or psychological issues related to their palliative diagnosis
- Management of complex social or family issues related to their palliative diagnosis
- Planning complex end of life care

Community Nursing Support:
Contact via STRATA referral form for:
- Personal Care and needs assessment (including funded end of life care at home and inbedded placements)
- Syringe drivers
- Continence aids
- Pressure sore management
- Overnight care via Marie Curie Nursing Care (community nurses will refer to this)
