Diabetes Remission

 

NHS Type 2 Diabetes Path to Remission Programme delivered by Counterweight. Is an evidence-based 12-month weight management programme including an initial total diet replacement (TDR) phase for weight loss. The programme is based on the DiRECT trial and is designed to support people diagnosed with type 2 diabetes to achieve:

  • Significant weight loss
  • Improvement in HbA1c
  • Reduction in medication and potentially achieve diabetes remission

 

The medically supervised programme provides patients with specialist support (dietitians, nutritionists, psychologists, medical team). Each phase of the programme is designed to support patients in building sustainable long-term behavioural change, weight loss and weight maintenance. The service is tailored to the patient and can be delivered either face-to-face or remotely (video call/phone call). The delivery is supported by educational content and tools provided via the Counterweight App or hardcopy workbook.

 

Eligibility criteria for the NHS Path to Remission programme

  • Aged between 18 and 65 years old
  • Confirmed diagnosis of type 2 diabetes within the last 6 years
  • BMI over 27 kg/m2 (White ethnic groups) or over 25 kg/m2 (Black, Asian and other
  • ethnic groups)
  • HbA1c measurement taken within the last 12 months

1. On diabetes medication 43-87 mmol/mol

2. Not on diabetes medication 48-87 mmol/mol
