Education for Type 2 Diabetes

Kernow Health has been unable to deliver DESMOND in Cornwall and the Isles of Scilly since the start of COVID-19 pandemic restrictions. NHS Kernow is currently in the process of transiting to a new, locally developed and delivered model of Type 2 diabetes structured education known as ‘Diabetes and you’. Healthy Living

Healthy Living is the first nationally commissioned digital structured education programme. It is an evidence-based, self-directed, web-based programme, and employs behaviour change models and nudge techniques to support users to take action. It provides users with a variety of tools and information to help them better manage their type 2 diabetes, live a healthy life and achieve good wellbeing.

Practices can refer participants to the programme by providing the registration page: https://www.healthyliving.nhs.uk/ where the individual can create an account.

Diabetes UK Learning Zone

The Diabetes UK Learning Zone gives advice on what to eat and tips for managing diabetes day-to-day. There are videos, quizzes and interactive tools tailored to the individual. All courses are completely free.

Diabetes UK also have a new and exciting partnership with Sport England called ‘Live Well Move More’ who are supporting people to become more active. There is a free 10 week online course, exercise hub and physical activity helpline. Email livewellmovemore@diabetes.org.uk  for more information.
