NHS Cornwall and the Isles of Scilly Talking Therapies (previously known as Outlook South West).

Referral criteria

Aged 16yr or older, registered with a GP in CIOS experiencing:

• Depression
• Stress
• Anxiety, including panic and phobic disorders
• Obsessions and compulsions
• Single-episode trauma

Physical long-term health conditions impacting on mental health (LTC IAPT), including:

• Diabetes and pre-diabetes
• CVD
• IBS
• COPD

Not exhaustive.

 

Exclusions

• Under specialist mental health services.
• Immediate risk to self or others, including suicidality.
• Substance misuse as main concern.
• Sexual violence as main concern.

 

Required information prior to referral

• Whether referral is for long-term health condition (LTC IAPT)
• Best patient contact details, including phone number and/or email
• Previous name, if applicable

 

Referral process

Patients can self-refer by phoning (01208) 871 905 or online

HCP can refer by completing the referral form

Referrals will be triaged.
