Urgent referral criteria
Aged 18yr to 75yr:
- Requiring specialist mental health assessment or intervention.
- Potential risk (not high or imminent) to self or others.
- Significant patient or carer distress or difficulty related to mental health condition.
Not exhaustive.
Routine referral criteria
Aged 18yr to 75yr:
- Requiring specialist mental health assessment or intervention.
- Stable with low risk to self or others.
- Diagnostic uncertainty, where specialist assessment is required to inform relevant treatment pathway.
- Other services able to support needs in the immediate term.
Not exhaustive.
Exclusions
- Imminent or high risk to self or others.

Required information prior to referral
- Consent.
- Up-to-date contact number for patient.
Referral process
Options to discuss case:
- Local PCN Primary Care Mental Health Integrated MDT - if available and time allows.
- SPoA, via Bodmin switch: (01208) 251 300.
To make a referral:
- Complete and email the ICMHT referral form to appropriate locality:
- East (North Cornwall and Caradon) – cft.eastreferralteam@nhs.net
- Mid (Restormel and Carrick) – cft.midreferralteam@nhs.net
- West (Kerrier, Penwith, and Isles of Scilly) –cft.westreferralteam@nhs.net
