Home Treatment Team
Service information
The Home Treatment Team (HTT) works with adult patients with acute mental health needs. They work with the patient to help manage and resolve difficulties in the patient’s own home. Where appropriate, they can also provide support for early discharge from hospital.
Patients on the HTT caseload will have 24/7 direct access to the HTT: day team is available 8am to 8:30pm with night cover from 8pm to 8:30am.
Inclusion criteria
- Patients aged over 18 years old who are experiencing an acute mental health difficulty, who without intensive support may further deteriorate and/or require hospital admission.

Exclusion criteria
- Children and adolescents – refer to CAMHS.

Mental health crisis:
- Imminent risk to self or others – consider calling 999.
- High risk to self or others – call 24/7 HCP Line and Crisis Hubon (01209) 881 999.

- In hours: Refer via SPoA for CMHT guideline.
- Out of hours: Contact Bodmin switchboard on (01208) 251 300 and ask for duty CPN.

Patients/family/carers:
Self-referrals can be made via NHS 111 (select option 2 for mental health support).
